Apply With Aadhaar Card.
Step1: Go to the official NDHM
Step2: Click to create your Health ID now.
Step3: Generate your health ID with Click Generate Via Aadhaar.
Step4: Fill your Aadhaar card number and tick both boxes and click Submit.
Step5: Enter OTP, and click submit.
Step6: Now, Enter your Mobile no. and click submit.
Step7: Enter OTP and submit.
Step8: See your Profile, create PHR Address, fill Email ID and click on Submit.
Step9; Now, your Health ID card has been generated, see your all details.
Step10: Click on Download health ID card.
